患者，男，50岁，因“咳嗽咳痰半月”于2024年3月5日收入上海市宝山区中西结合医院呼吸危重症医学科住院治疗。患者咳嗽咳痰，少量黄痰，偶有气短。体温36.7℃，脉搏102次/min，呼吸18次/min，血压118/76mm Hg(1 mmHg=0.133 kPa)。口唇稍紫，咽部正常，扁桃体无肿大。气管居中，双侧甲状腺无肿大，颈静脉无怒张，胸廓正常。两肺呼吸音粗，未及明显干湿啰音及哮鸣音，未闻及胸膜摩擦音。根据体格检查、专科检查、辅助检查等，确诊为亚洲诺卡菌所致肺部感染。给予早期、足量、足疗程的精准用药后患者病情好转。患者肺炎基本吸收，无明显呼吸道症状，病情稳定，生活完全自理，与家人相处融洽，可正常工作。
Abstract：A 50-year-old male patient was admitted to the Department of Respiratory and Critical Care Medicine at Shanghai Baoshan District Hospital of Integrated Traditional Chinese and Western Medicine on March 5, 2024, due to “cough with sputum for half a month”. The main symptoms included cough with a small amount of yellow sputum and occasional shortness of breath. His body temperature was 36.7 ℃, pulse rate 102 beats/min, respiratory rate 18 breaths/min, and blood pressure 118/76 mmHg. The patient presented with slightly cyanotic lips, normal pharynx, no tonsillar enlargement, midline trachea, no thyroid enlargement, no jugular vein distension, and normal chest shape. Bilateral lung auscultation revealed coarse breath sounds without obvious dry or wet rales, wheezing, or pleural friction rub. Based on physical examination, specialized examinations, and auxiliary tests, the patient was diagnosed with pulmonary infection caused by Nocardia asiatica. The patient was treated with early, adequate, and full-course precision medication, leading to gradual improvement in his condition. The pneumonia was largely resolved, with no significant respiratory symptoms. The patient's condition remained stable, and he was fully capable of self-care, maintained good relationships with family, and was able to resume normal work.
