目的 探讨超声监视下新型控制性水灌肠整复术在小儿肠套叠治疗中的临床应用效果。方法 本研究采用随机对照试验,选取2023年11月—2024年6月重庆市涪陵区妇幼保健院收治的肠套叠患儿150例,随机分为新型控制性水灌肠组、传统水灌肠组和空气灌肠组,各50例。所有患儿均接受相应的灌肠整复术治疗,观察患儿的复位成功率、复位时间、肠道注水量及并发症发生率等指标。结果 新型控制性水灌肠组的复位成功率(96.00%)显著高于传统水灌肠组(80.00%)和空气灌肠组(86.00%)(χ2=5.562,P=0.018);新型控制性水灌肠组的复位时间(18.50±2.50)min显著短于传统水灌肠组(24.56±3.54)min和空气灌肠组(22.00±3.02)min(F=10.232,P<0.001)。新型控制性水灌肠组的肠道注水量(1203.01±50.03)mL显著少于传统水灌肠组(1450.30±70.30)mL和空气灌肠组(1351.02±62.12)mL(F=15.452,P<0.001);新型控制性水灌肠组的并发症总发生率(8.00%)显著低于传统水灌肠组(24.00%)和空气灌肠组(18.00%)(χ2=4.560,P=0.033)。结论 超声监视下新型控制性水灌肠整复术在小儿肠套叠治疗中具有显著优势,复位成功率高,复位时间短,肠道注水量少,并发症发生率低。
Abstract:Objective To investigate the clinical application of novel controlled water enema under ultrasonic monitoring in the treatment of pediatric intussusception. Methods A total of 150 children with intussusception admitted to our hospital from November 2023 to June 2024 were randomly divided into three groups: novel controlled water enema group, traditional water enema group, and air enema group, with 50 cases in each group. All children received the corresponding enema reduction methods, and the reduction success rate, reduction time, intestinal irrigation volume, and complication rate were observed. Results The reduction success rate in the novel controlled water enema group(96.00%)was significantly higher than that in the traditional water enema group(80.00%)and the air enema group(86.00%)(χ2=5.562,P=0.018). The reduction time in the novel controlled water enema group(18.50±2.50)min was significantly shorter than that in the traditional water enema group(24.56±3.54)min and the air enema group(22.00±3.02)min(F=10.232, P<0.001). The intestinal irrigation volume in the novel controlled water enema group(1203.01±50.03)mL was significantly less than that in the traditional water enema group(1450.30±70.30)mL and the air enema group(1351.02±62.12)mL(F=15.452, P<0.001). The total complication rate in the novel controlled water enema group(8.00%)was significantly lower than that in the traditional water enema group(24.00%)and the air enema group(18.00%)(χ2=4.560, P=0.033). Conclusion The novel controlled water enema under ultrasonic monitoring has significant advantages in the treatment of pediatric intussusception, with high reduction success rate, short reduction time, low intestinal irrigation volume, and low complication rate.
