目的 了解徐州市贾汪区人民医院糖尿病患者降糖及护肾药物使用情况，分析临床药师药学服务情况与效果，以促进医院药学服务发展。方法 收集2022年1月—2024年7月徐州市贾汪区人民医院收治的411例糖尿病肾病患者的临床信息和药学服务记录，统计分析患者的临床基础信息和生化指标，深入探究患者的药物使用情况及药学服务成效。结果 从收集的123例门诊患者及288例住院患者临床信息可知，患者疾病主要以2型糖尿病肾病Ⅴ期为主；降糖药主要分为口服降糖药、胰岛素及其类似物；护肾药主要分为非甾体醛固酮受体拮抗剂与中成药制剂；药学服务内容主要为联合使用合理性，其次为药物重整。结论 临床药师的药学服务在糖尿病肾病患者的治疗过程中发挥着重要作用，能够保障用药合理性，并提升药师价值。

Objective To investigate the use of hypoglycemic and nephroprotective medications among diabetic patients at Jiawang District People's Hospital in Xuzhou City, and to analyze the provision and outcomes of clinical pharmacist services to promote the development of hospital pharmacy services. Methods Clinical information and pharmaceutical service records of 411 diabetic nephropathy patients admitted to Jiawang District People's Hospital, Xuzhou City, from January 2022 to July 2024 were collected. Statistical analysis was conducted on patients' clinical baseline information and biochemical indicators to investigate medication usage patterns and the effectiveness of pharmaceutical services. Results Clinical data from 123 outpatients and 288 inpatients revealed that most patients had stage Ⅴ type 2 diabetic nephropathy. Antidiabetic drugs were primarily categorized as oral hypoglycemic agents, insulin and its analogues; Nephroprotective agents were primarily nonsteroidal aldosterone receptor antagonists and traditional Chinese medicine formulations. Pharmaceutical services primarily focused on rational combination therapy, followed by medication reconciliation. Conclusion Pharmacist-provided pharmaceutical services play a crucial role in the treatment of diabetic nephropathy patients, ensuring rational medication use and enhancing the value of pharmacists.
