目的 探讨基于营养风险评估(nutritional risk screening,NRS)的个性化营养干预对老年认知障碍患者的影响。方法 选取无锡市精神卫生中心2023年1月—2024年12月收治的80例老年认知障碍患者,根据随机数字表法分为观察组和对照组,各40例。对照组接受常规饮食干预,观察组在常规饮食干预基础上接受个性化营养干预。持续干预6个月后,采用NRS-2002评分、简易精神状态评估量表(mini-mental state examination,MMSE)评分、日常生活活动能力(activities of daily living,ADL)评分、血液生化指标和生活质量评估量表(short form-36,SF-36)评估干预效果。结果 干预后,观察组NRS-2002评分、体质指数(body mass index,BMI)、白蛋白、前白蛋白、肌酐及体重变化均优于对照组(P<0.05),MMSE评分和ADL评分高于对照组(P<0.05);干预后,观察组的C反应蛋白(Creactive protein,CRP)、白介素-6(interleukin-6,IL-6)和总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)白水平均显著低于对照组(P<0.05);干预后,观察组在生理功能、心理健康及社会适应性方面的生活质量显著高于对照组(P<0.05)。结论 基于营养风险评估的个性化营养干预能够显著改善老年认知障碍患者的营养状况、认知功能、炎症水平及生活质量,具有较好的临床应用价值。
Abstract:Objective To explore the impact of personalized nutritional intervention based on nutritional risk screening(NRS)on elderly patients with cognitive impairment. Methods 80 elderly patients with cognitive impairment admitted to Wuxi Mental Health Center from January 2023 to December 2024 were selected and divided into the observation group and the control group according to the random number table method, with 40cases in each group. The control group received conventional dietary intervention, while the observation group received individualized nutritional intervention on the basis of conventional dietary intervention. After 6 months of continuous intervention, the NRS-2002 score, the mini-mental state examination(MMSE)score, and the activities of daily living were adopted. The intervention effect was evaluated by ADL score, blood biochemical indicators and the short form-36(SF-36)quality of life assessment scale. Results After the intervention, the NRS-2002 score, body mass index(BMI), albumin, prealbumin, creatinine and weight changes in the observation group were all better than those in the control group(P<0.05), and the MMSE score and ADL score were higher than those in the control group(P<0.05). The C-reactive protein(CRP), interleukin-6(IL-6)and total cholesterol of the observation group after intervention The average values of TC and low-density lipoprotein cholesterol(LDL-C)in white water were significantly lower than those in the control group(P<0.05). After the intervention,the quality of life of the observation group in terms of physiological function, mental health and social adaptability was significantly higher than that of the control group(P<0.05). Conclusion Personalized nutritional intervention based on nutritional risk assessment can significantly improve the nutritional status, cognitive function,inflammatory levels, and quality of life in elderly patients with cognitive impairment, demonstrating good clinical application value.
